MCL LIGAMENT INJURY

Definition/Description
A medial collateral ligament (MCL) injury is a stretch, partial tear, or complete tear of the ligament on the inside of the knee. It is one of the most common knee injuries and results mostly from a valgus force on the knee

EPIDEMIOLOGY;
MCL injuries mostly occur after an impact on the outside of the knee, lower thigh or upper leg, when the foot is in contact with the ground, and unable to move. The MCL on the inside of the knee will become stressed due to the impact, and a combined movement of flexion/valgus/external rotation will lead to tears in the fibres. The athlete might feel immediate pain, and feel or hear a popping or tearing sound. Mostly the deep part of the ligament gets damaged first, and this may lead to medial meniscal damage or anterior cruciate ligament damage.

 *Definition:

Sclerodactyly is a hardening of the skin of the hand that causes the fingers to curl inward and take on a claw-like shape. It is brought on by a condition called systemic scleroderma, or systemic sclerosis.

Systemic scleroderma often affects the hands, causing a tightening or hardening of the skin. But not everyone with scleroderma symptoms in the hands will get sclerodactyly.
Scleroderma is a form of arthritis. It is a rare disease. Fewer than a half million people in the United States are affected, and only some of these get sclerodactyly.

*symtoms:

As the scleroderma of the hand progresses, your fingers can become so swollen that it’s hard to bend them.
In some cases, the skin may then thicken and become hard.
If it reaches the point that the hand becomes stiff like a claw, and the fingers immovable, it is then sclerodactyly.
There is usually also a wasting away of the underlying soft tissues.

 *causes:Scleroderma is considered an autoimmune disease.

 Your immune system mistakes your own tissue for a foreign body and attacks it.
Your cells respond to the mistaken attack by over-producing a protein called collagen. Collagen is found in healthy skin, and makes it both supple and firm. But when too much collagen is produced, your skin can get hard and leathery.
There is some evidence that scleroderma, the cause of sclerodactyly, may be hereditary. Exposure to some pesticides, epoxy resins, or solvents may trigger scleroderma in certain people.

How does sclerodactyly progress:

One of the early signs of scleroderma is extreme sensitivity to cold in the fingers or toes.
Your fingers may feel numb or painful. This can also be brought on by emotional stress.
As scleroderma progresses, the fingers and toes may swell and appear puffy.
The initial phase of scleroderma in the hands may be intermittent:

*It may last for weeks, months, or years.

*It is often worse in the morning.

*Over time, the skin may thicken and then harden.

*The skin on your hands can become so tight that there’s nothing there to pinch.

 Treatment:

it is very important to have treatment in the initial stages before the hands have hardened.
Treatment options for sclerodactyly include physical and occupational therapy, ultraviolet light, and surgery.

Childhood Apraxia of Speech Causes, Symptoms.


 Childhood apraxia of speech (CAS) is a speech disorder in which a child's brain has difficulty coordinating the complex mouth movements required to form sounds into syllables, syllables into words, and words into phrases. Usually, muscle weakness does not cause this speech disorder.

  Parents of children with childhood apraxia of speech commonly say, "Nobody can understand my son," "He looks like he's trying to say a word but can't get it out," and "He said that one word. Time, and then I didn't hear it again."

  The most common description of a child with childhood apraxia of speech is that he has difficulty understanding.
 An accurate diagnosis of childhood apraxia by a speech-language pathologist (SLP) requires a comprehensive speech and language evaluation. An SLP assesses your child's speech skills and expressive and receptive language abilities while gathering information from your family about how your child communicates at home and in other situations.

  It is important that the SLP who evaluates your child has experience and expertise in diagnosing and working up childhood speech apraxia so that an accurate diagnosis can be made and other diagnoses can be ruled out. For example, childhood apraxia of speech is often confused with a severe speech disorder because both diagnoses involve poor speech intelligibility. Unfortunately, the approach taken to address a speech disorder is very different from the approach to childhood apraxia of speech, and confusing the two can reduce treatment gains.

  An assessment of childhood apraxia of speech must include an evaluation of your child's expressive and receptive language abilities; Many children with this disorder exhibit deficits in their language skills. Additionally, gaps between receptive and expressive language skills, word order confusion, and difficulty recalling words are common in children with apraxia of speech. A thorough assessment of your child's abilities is necessary so that treatment goals can be developed based on his individual needs.

What is Eating Disorder?


Eating disorder can shown in behavioral conditions characterized by severe and persistent disturbance in eating behaviors and associated distressing thoughts and emotions. It can be very serious conditions affecting physical, psychological and social function. 

Types of eating disorders:-

There are many types that is include anorexia nervosa, bulimia nervosa, binge eating disorder, avoidant restrictive food intake disorder, other specified feeding and eating disorder, pica and rumination disorder.

Symptoms:-
Symptoms may vary, depending on the type of eating disorder. People with eating disorders can have all different body types and sizes.

Causes:-
The exact cause of eating disorders is not known. As with other mental health conditions, there may be different causes, such as 

1.Genetics:- Some people may have genes that increase their risk of developing eating disorders.
2.Biology:-Biological factors, such as changes in brain chemicals, may play a role in eating disorders.

Complications:-
Eating disorders cause a wide variety of complications, some of them life-threatening. The more severe or long lasting the eating disorder, the more likely it is that serious complications may occur. 

These may include:

1.Serious health problems.
2.Depression and anxiety.
3.Suicidal thoughts or behavior.
4.Problems with growth and development.
5.Social and relationship problems.
6.Substance use disorders.
7.Work and school issues.
8.Death.

Prevention:-
There's no sure way to prevent eating disorders, but you can take steps to develop healthy eating habits. If you have a child, you can help your child lower the risk of developing eating disorders.

In stepwise process:-
* choose a healthy diet
* don't use dietary supplements laxatives like herbal products
*make physical activity like aerobica each week min 150 mins
* cinsult a doctor for depression, anxiety and any other mental health issues

GUYON CANAL 

Guyon canal syndrome is a relatively rare peripheral ulnar neuropathy that involves injury to the distal portion of the ulnar nerve as it travels through a narrow anatomic corridor at the wrist. The ulnar nerve originates from C8-T1 and is a terminal branch of the brachial plexus. In the upper arm, the ulnar nerve courses posterior and medial to the brachial artery, and heads for the posterior aspect of the elbow, piercing the medial intermuscular (IM) septum at the arcade of Struthers. The nerve pierces the medial IM septum approximately 8 cm proximal to the medial epicondyle.

The ulnar nerve enters the cubital tunnel posterior to the medial epicondyle and enters the forearm by piercing between the two heads of the flexor carpi ulnaris (FCU) muscle. The ulnar nerve reaches the hand via Guyon canal to provide motor and sensory innervation to the digits. Guyon canal is a unique location where the ulnar nerve is vulnerable to compressive injury, although the more common location of the ulnar nerve injury occurs at the elbow which is known as cubital tunnel syndrome.

The anatomic boundaries of Guyon canal include:

Volar carpal ligament - the "roof"
Transverse carpal ligament - the "floor"
Note that the transverse carpal ligament spans Guyon canal as the floor at the ulnar side of the hand/wrist before seamlessly transitioning to its position as the "roof" of the carpal tunnel
Pisiform, Pisohamate ligament, abductor digiti minimi - ulnar boundary
The hook of hamate - radial boundary

Etiologies include:

Ganglion Cyst
The hook of hamate Fracture/Displacement
Tumors (e.g., lipoma)
Repetitive trauma (e.g., Cyclist's handlebar external compression)
Aberrant Muscle (e.g., abductor digiti minimi) or excess fat tissue within the canal
Ulnar artery thrombosis or aneurysm (e.g., Hypothenar Hammer Syndrome)

MANAGEMENT
Wrist splinting must keep the wrist in a neutral position but may allow fingers to move around freely. A splint is to be worn at least at night for a recommended duration of one to 12 weeks.

HUMAN RESOURCE IN DISABILITY SECTOR

STATUS OF HUMAN RESOURCES IN INDIA:
                   India is one of the few countries in the world where the development of teachers for the education of persons with disabilities (Divyangajan) does not come under the purview of the Ministry of Human Resource Development.  It is responsible for the Department of Empowerment of Persons with Disabilities (Divyangajan), Ministry of Social Justice and Empowerment, whose main focus is rehabilitation, not teacher development.  According to 2011 census, there are 268 lakh persons with disabilities in India.  Majority of this population lives in rural areas and still misses out on rehabilitation services due to lack of trained rehabilitation professionals.  The Right to Free and Compulsory Education Act 2010 talks about minimum 1:30 ratio of teachers and students;  But the Rehabilitation Council of India (RCI) has till now trained around one lakh professionals in total 16 disciplines from certificate to Ph.D.  level courses.  On the one hand urban India struggles for quality of trained rehabilitation professionals.  Parents and family members complain that the quality of professionals is deficient.  On the other hand, the profession is not attracting good talent as trained professionals complain of not being employed.  The recently passed Rights of Persons with Disabilities Act 2016 added 14 more types of disabilities.  Therefore, many other categories of professionals are needed to serve these added categories.  This present study attempts to discuss these burning issues and other challenges.  This study also tries to bring some insight into these issues which can change the future course of action for human resource development in the field of disability rehabilitation.

As a parent, you want your children to learn all they can—grasp math concepts, be curious about exploring the world, and learn to read and write. Did you know that playing with your son or daughter can help them study? Playing and learning go hand in hand!

  What kind of play helps children learn best? A game that really engages kids—play that keeps them focused and stays with them even when problems arise. This type of play helps children develop their approaches to learning—in other words, the ways they respond to learning situations. Curiosity about the world, initiative and problem solving, and focus and persistence are some of the learning styles that children develop through play.

  In the early years, parents can help children develop skills to be better students by playing with them. Yes, as they enter kindergarten and elementary years, children should have some understanding of letters and numbers. However, if they do not develop strong approaches to learning, they will not be successful in school settings.
 Encouraging toddlers in play

  Joey is 20 months old. He has a basket of toys, including a rattle, soft plastic blocks, a set of stocking rings, stuffed animals, and cloth and plastic books. Joey's father often invites him to sit on the floor with Joey and play with the items in the basket. Joey's favorite activity is to dump all the toys and put the basket on his head! This is normal toddler play behavior. Joey is curious about the world and sees it in a different way through the tiles in the basket!

  Joey loves shaking the rattle or stacking two or three blocks and knocking them over to hear different sounds. Each may have his attention span of five minutes or more, which is appropriate for his age. He can solve problems as he tries to place the rings on the stacking post or add more blocks to the tower.

  Joey's father encourages his curiosity. He commented on what he was doing: "I see you're trying to get the last ring in the post, but it's not enough." Or he asks him questions: “Where did that ball go? Do you see it hiding behind the chair?" He linked his play to practice by responding positively to his interest: "I can tell you to look through the basket, silly boy. Does everything look different from there?" He encourages him by asking him to keep trying even when he feels down. “Oh, those blocks keep falling, don't they? Can you try to put one on top of the other? Let's see what happens. I'll help you." This encouragement encourages his persistence, his attention and his initiative in problem solving, all positive approaches to learning.
