Scope & limits
Workbook entries are company-supplied, not live government registry checks. The RS column is undefined; its ACTIVE/INACTIVE flags do not establish current operational or HFR status. HFR is not clinical-outcome certification or a replacement for local permissions. A matching ID does not independently reconcile current addresses.
The workbook’s Tirupati entry is IN2810065148. The separately inspected IN2810053847 certificate is retained as an additional record; replacement, cancellation or address equivalence has not been established. Duplicate Srikakulam certificate copies count once.
