Topic: TREATMENT FOR MILLER FISHER SYNDROME 

DEFINATION:

-Miller Fisher syndrome (MFS) is a rare acquired nerve disease related to Guillain-Barré syndrome (GBS).

-MFS is rare, affecting one to two people per million each year. It is an autoimmune disease, in which the immune system attacks the nerves.

CAUSES: 

-MFS is an autoimmune disease in which antibodies against a bacterial or viral infection cross-react with and attack the nerves. The site of attack may be the myelin sheaths, which insulate and protect the nerve fibers (axons), or the axons themselves.

-MFS does not usually affect more than one person in the same family. There are very rare reports of siblings or identical twins being affected. It is thus possible that there is a hereditary predisposition to develop the disease. It is not infectious.

SIGNS AND SYMPTOMS:

MFS has three defining features:

-ophthalmoplegia (eye muscle weakness resulting in impaired eye movements and consequent double vision)

-ataxia (incoordination of the limbs)

-areflexia (absence of tendon reflexes)

-These symptoms typically develop rapidly over a few days. Some patients have weakness of the face, tongue and swallowing muscles as well. Others also develop weakness of the limbs and breathing muscles and are then considered to have GBS-MFS overlap syndrome. MFS often occurs several days or up to four weeks after an infective illness.

DIAGNOSIS:

-The diagnosis of MFS is clinical. It depends on identifying the characteristic features from the symptoms and physical examination. There is no definitive diagnostic test.

-May be MRI and Nerve conduction test can support the diagnosis.

TREATMENT:

Most patients recover within six months. The average recovery time is 8 to 12 weeks. The disease rarely causes permanent neurological problems. Relapse occurs in less than 3 percent of patients. Death is very rare. Supportive care to manage the physical and emotional challenges of the disease is needed. Good nursing, physical therapy, occupational therapy and psychological support are important.

A student with a disability register with the office that provides support services for students with disabilities, in order to receive accommodations. It is the student's responsibility to request services in a timely manner. These offices confirm the student's disability and eligibility for services and accommodations. A course instructor typically receives a letter from this office detailing recommendedaccommodations for a student. The student with a disability is responsible for meeting all course requirements using only approved accommodations.

The goal is to give the student with a disability equal access to the learning environment. Individualized accommodations are not designed to give the student an advantage over other students, to alter a fundamental aspect of the course, nor to weaken academic rigor.specific learning disability is unique to the individual and can be manifested in a variety of ways. 

To show what they know without giving them an unfair advantage. Accommodations are divided into the following categories: how information is presented to the student, how the student can respond, timing of tests and lessons, the learning environment, and test scheduling.
Accommodations are alterations in the way tasks are presented that allow children with learning disabilities to complete the same assignments as other students. Accommodations do not alter the content of assignments, give students an unfair advantage or in the case of assessments, change what a test measures. They do make it possible for students with LD to show what they know without being impeded by their disability.
Once a child has been formally identified with a learning disability, the child or parent may request accommodations for that child's specific needs. The Individuals with Disabilities Education Act states that a child's IEP team which both parent and child are a part of must decide which accommodations are appropriate for him or her. Any appropriate accommodations should be written into a student's IEP.School assignments and tests completed with accommodations should be graded the same way as those completed without accommodations. After all, accommodations are meant to level the playing field, provide equal and ready access to the task at hand, and not meant to provide an undue advantage to the user.assist the student in finding effective peer note-takers from the class. Alternatively, you could provide the student with a copy of your lecture notes or outline.allow the student to tape record lecturesallow the student additional time to complete in-class assignments, particularly writing assignments.

provide feedback and assist the student in planning the workflow of assignments. This is especially important with large writing assignments. It may be helpful to break the larger assignment into smaller components with opportunities for draft feedback.
provide assistance with proofreading written work.
The term "accommodation" may be used to describe an alteration of environment, curriculum format, or equipment that allows an individual with a disability to gain access to content and/or complete assigned tasks. They allow students with disabilities to pursue a regular course of study. Since accommodations do not alter what is being taught, instructors should be able to implement the same grading scale for students with disabilities as they do for students without disabilities. Important to understand the strengths, the advantages, and limitations of each accommodation type and to offer a range of options to guests in order to provide the best possible experience and meet their individual needs.

Voice disorders can occur when our vocal folds are unable to properly vibrate. This can happen when vocal folds become inflamed; develop nodules, polyps, or other growths; or are unable to move properly. A voice disorder can affect the pitch, volume, or quality of your voice.

Symptoms of Voice Disorder:-

Hoarse, rough, or raspy voice

Weak or breathy voice
Changes to your voice’s pitch or volume

Raw or strained throat
Tension or pain in your throat while speaking
Difficulty talking or breathing

Repeatedly clearing your throat

The feeling of a lump in your throat when swallowing

Pain when touching the outside of your throat

Causes of Voice Disorder:-

Some causes of voice problems include:-

Vocal abuse: -  Vocal abuse is any type of behavior that can strain, harm, or injure your vocal folds. This can be caused by excessive talking or screaming, inhaling irritants, smoking, coughing, or clearing your throat. Vocal abuse can often lead to the development of nodules, polyps, or other growths on your vocal folds, which can change how your voice sounds. Frequently engaging in activities that cause vocal abuse and damage your vocal folds can have both temporary or permanent effects on your voice quality and vocal function.

Abnormal growths: - Some people may develop extra tissue on their vocal folds that can impact their function. These growths have a variety of causes, including injury, vocal abuse, cancer, or illness. These growths can take many forms, including nodules, polyps, cysts, papillomas, lesions, and more.

Nerve problems: - Your central nervous system controls your voice and swallowing abilities. There are several health conditions that can affect these nerves, including multiple sclerosis, Huntington's disease, Parkinson's disease, or amyotrophic lateral sclerosis.

Inflammation and swelling: - When your vocal cords become inflamed, they can affect your natural airflow. Inflammation and swelling of the vocal folds can be caused by surgery, respiratory illness, allergies, smoking, vocal abuse, substance abuse, and more.

Hormones:- Disorders that affect your hormones can cause voice disorders, including thyroid hormones and growth hormones.

Treatment of Voice Disorder:- 

Lifestyle changes: -There may be certain changes you can make to your lifestyle and behaviors that can help you control your voice disorder or reduce its symptoms. Common changes include lowering the volume of your voice, reducing yelling or speaking loudly, and resting your voice at regular intervals. There are also certain exercises that can help relax or relieve the tension in your vocal folds.

Medicines: - Medication can sometimes help the root cause of your voice disorder. Talk with your doctor before taking any medicines.

Injections: - Your doctor may recommend injections if muscle spasms are causing your voice disorder. There are other types of injections that can help control your vocal folds.

Surgery:- In some circumstances, your doctor may recommend surgery to remove nodules, polyps, or other types of growth on your vocal folds.

Voice therapy: - In many cases, your doctor will recommend voice therapy from a licensed speech therapist. This can be used as your primary treatment, alongside other types of treatments, or as a precursor to medical treatments. We’ve provided more information on voice therapy below.
